request your membername and password. If you do not want to remove your membership,
log in to your membership.
This form cannot cancel memberships that are still within a Paid Membership or the 30 Day Trial.
Membername
Enter the membername of the account you wish to remove. (e.g. joe123)
Password
Enter the correct password for the account. (e.g. xyz123) You must know the correct password to remove your account.
Email Address
Enter the email address for the membership. (e.g. jsmith@example.com) This would be the email address you used when you signed up for the membership, unless you changed it afterward. You must know the correct email address to remove your account.
Reason for removal
Enter, in your own words, your reason for cancelling your membership.
