I won't bother with the legal stuff, as it's different from state to state, but in general you must live full time for a specified period before the shrink signs you off. Then you're cleared for the sex reasignment surgery if you so wish. Many of us are happy with hormone replacement therapy alone, so we can keep our little friend.
